Survivorship plan

  1. Will I get a written survivorship care plan?
  2. Who is my primary care doctor moving forward — and what coordination is needed?
  3. Are there long-term side effects I should watch for?
  4. When can my port / catheter come out?
  5. Is there a survivorship clinic at this center?

Lifestyle moving forward

  1. Diet, exercise, weight — what specifically helps reduce recurrence?
  2. Alcohol, supplements, sun exposure — anything to avoid?
  3. When can I resume normal activity (sex, exercise, travel)?
  4. Mental health resources — what do you recommend at this stage?
